CRITÉRIOS DE INDICAÇÃO DE TOMOGRAFIA EM CRIANÇAS QUE SOFRERAM TRAUMATISMO CRANIOENCEFÁLICO LEVE: UM ESTUDO TRANSVERSAL

Resumo

INTRODUÇÃO: Para evitar a exposição inoportuna à radiação da tomografia computadorizada (TC) em crianças que sofreram traumatismo cranioencefálico (TCE) leve, são utilizados protocolos que guiam a decisão médica na indicação desse exame. Portanto, o objetivo do presente estudo foi avaliar os critérios de indicação de TC em crianças que sofreram TCE leve no pronto atendimento de um hospital de alta complexidade. METODOLOGIA: Neste estudo, foram coletados dados epidemiológicos e clínicos de 328 prontuários de crianças com menos de 14 anos, diagnosticadas com TCE leve que realizaram TC. Além disso, foi realizada a aplicação do protocolo Pediatric Emergency Care Applied Research Network (PECARN), que indica a realização de TC ou não. RESULTADO: A partir da análise dos dados coletados, 55,8% eram do sexo masculino, com idade média de 2 anos. Os acidentes ocorreram mais em ambiente residencial e os principais sintomas apresentados foram vômitos, sonolência, choro excessivo e perda de consciência. A TC foi indicada pelo PECARN em 34,5% dos casos, sendo que 5,2% dos pacientes apresentaram alterações na TC. CONCLUSÃO: Esse estudo identificou que crianças do sexo masculino foram mais acometidas por TCE, com uma mediana de 2 anos de idade. Poucos pacientes apresentaram sintomas, e o mais prevalente foi o vômito. De todos os exames realizados, o protocolo PECARN indicou apenas um terço deles. Ademais, apenas 5,2% dos exames vieram alterados. Por fim, aqueles com a idade < 3 anos e com indicação da TC pelo protocolo foram os que mais apresentaram alterações na TC.
